Effective May 1, 2024, Health Services & Benefit Administrators (HS&BA)
will be the contract administrator for the Trust Funds.

1095-B Proof of Coverage Available Upon Request 

Due to changes in federal law, 1095-B forms are no longer required to automatically

be mailed to individuals, but must be provided upon request.
